USODA 2004 Atlantic Coast Optimist Championship RegistrationEvents/2004/10.09.junior/ACCMedicalForm.pdfRegistration RequirementsIn order to register, you will need to fill out an Entry Form Word - 26Kb or pdf- 105Kb and pay the $80 entry fee, and complete a Medical Form and Waiver Word- 16Kb or pdf- 113Kb . An additional late registration fee of $50 will be imposed for entries on line after 5:00 pm October 4, or postmarked after September 30, by mail. No INITIAL registrations will be allowed after 9:00 pm on Friday October 8. Check-in of PRIOR registrants will be allowed during Registration on Saturday, October 9, but such late check-in is discouraged.You can do everything by mail if you wish. Download the Entry Form and Medical Form and Waiver and send them in along with your check. On Line RegistrationWe have just installed our On-line Registration and it is now available for use. You may also fill out almost everything on line, and process the payment online. If you do that, you will have met the deadline for registration and payment, and you can send in the Entry Form and Medical Form and Waiver in due course, or even submit it at the Registration Desk when you come to Fishing Bay.The On Line ProcessBefore you begin, please make sure you have a credit card ready to perform the final step, which is the payment of the $80 entry fee. You will also be offered the meal packages plus event tee shirts. As to meals, the entry fee covers breakfast and lunch (but not dinner) for the competitors but no meals for anyone else (like parents). FBYC is rather isolated, and eating at the club is certainly the easiest way to eat at the event, and we hope you will sign up now so we can plan the numbers of meals.Please note that online registration has now closed for this event.
